This Policy does not replace our HIPAA Notice of Privacy Practices, which governs how we use/disclose protected health information (PHI) in the course of providing therapy services.

1) Treatment
To provide, coordinate, or manage your care (including consultation with other providers you authorize or when permitted).
2) Payment
To bill and collect payment (insurance, EAPs, etc., if applicable).
3) Health Care Operations
Quality assessment, business management, audits, training, legal services, and certain administrative activities.

Get a copy of your PHI (with limited exceptions)
Request corrections to your PHI
Request confidential communications (e.g., alternative address/phone)
Ask us to limit what we use/disclose (we are not always required to agree, except in certain cases)

Choose someone (personal representative) to act for you when legally authorized
If you believe your privacy rights were violated, you may file a complaint with:

We will not retaliate against you for filing a complaint.

We use electronic systems to maintain clinical records and communicate securely.
