This Policy does not replace our HIPAA Notice of Privacy Practices, which governs how we use/disclose protected health information (PHI) in the course of providing therapy services.

Important: Please avoid submitting highly sensitive personal or clinical information through general website forms unless the form clearly states it is part of a secure client portal/intake process.

This Notice applies to the protected health information (“PHI”) we create or receive about you while providing therapy services.

A. Our Legal Duties

We are required by law to:

  • Maintain the privacy of your PHI

  • Provide you with this Notice of our legal duties and privacy practices

  • Follow the terms of the Notice currently in effect

B. How We May Use and Disclose Your PHI (Without Your Written Authorization)

HIPAA generally permits uses/disclosures for:

1) Treatment
To provide, coordinate, or manage your care (including consultation with other providers you authorize or when permitted).

2) Payment
To bill and collect payment (insurance, EAPs, etc., if applicable).

3) Health Care Operations
Quality assessment, business management, audits, training, legal services, and certain administrative activities.

Other permitted/required disclosures may include:

  • As required by law

  • Public health activities (where applicable)

  • Health oversight activities

  • Serious threat to health/safety

  • Abuse/neglect reporting (as required)

  • Judicial/administrative proceedings (with appropriate safeguards)

  • Law enforcement (limited circumstances)

  • Coroners/medical examiners (where applicable)

C. Uses and Disclosures Requiring Your Written Authorization

We will obtain your written authorization for uses/disclosures not described in this Notice, unless otherwise permitted by law. You may revoke an authorization in writing (except to the extent we already acted on it).

D. Special Rules: Reproductive Health Information Requests (HIPAA Final Rule)

Federal rules include strengthened protections related to reproductive health information, including requiring an attestation for certain requests and limiting use/disclosure for prohibited purposes. We will follow applicable requirements and update our practices accordingly. HHS+1

E. Your Rights Regarding Your PHI

You have the right to:

  1. Get a copy of your PHI (with limited exceptions)

  2. Request corrections to your PHI

  3. Request confidential communications (e.g., alternative address/phone)

  4. Ask us to limit what we use/disclose (we are not always required to agree, except in certain cases)

  5. Get a list (“accounting”) of certain disclosures

  6. Get a copy of this Notice at any time

  7. Choose someone (personal representative) to act for you when legally authorized

F. How to File a Complaint

If you believe your privacy rights were violated, you may file a complaint with:

  • The U.S. Department of Health and Human Services (HHS), Office for Civil Rights.

We will not retaliate against you for filing a complaint.

G. Texas Notice Regarding Electronic Disclosure (Chapter 181)

Texas law includes notice requirements related to electronic disclosure of PHI and provides for general notice methods such as posting notice in a place of business. Texas Statutes+1


Our practice provides notice and obtains authorization when required under Texas law.

H. Telehealth

I. Our Use of Electronic Health Records and Vendors

We use electronic systems to maintain clinical records and communicate securely.

J. Changes to This Notice

We may change this Notice. The revised Notice will apply to PHI we already have and any PHI we receive in the future. The current Notice will be available on our website and upon request.
